Problem: there's some kind of glitch where sometimes if you fire a grenade while running it vanishes.

If this happens it means you have found a seam between map sectors. I tried to make the seams invisible, but they occasionally cause entity resets (loss of grenades).
There are no seams in the Challenge Maps.
